We make sure you receive the care you need so you can live a comfortable life in the comfort of your own home. This includes activities such as showering, grooming, dressing, medication assistance and food preparation etc.  We ensure respectful in-home support is tailored to your daily routine, preferences and goals. 

Social participation is an essential part of living a full and independent life. We provide tailored support to help you connect with your community, build confidence and enjoy the things you love in your way and at your own pace.

Including supported shopping, medical appointments, sporting and recreational events, social activities, visiting or making new friends, travel and transport and building confidence and social skills. We also provide assistance with travel and transport. 

Cardinia Community Care provides support coordination to NDIA managed, plan managed and self-managed clients. We also provide care coordination to clients with an aged care package.  

Your Coordinator will help you to negotiate with disability and aged care service providers about what they can offer you and how much it will cost out of your plan or package. They will ensure you find support that meets your needs, no matter how complex. Your coordinator will make sure that service agreements and bookings are completed to help build your ability to exercise choice and control over your plan.
